When i gave birth to my second child last Feb 2013, after receiving the registered Birth Certificate from the Hospital, I instantly started the process in securing her a passport as i need to bring her back to Singapore before she turned 2 months. I called the Department of Foreign Affairs (DFA) Cebu Office and asked them what are the documents required. They informed me to bring the infant Birth Certificate in Security Paper (SECPA) issued by the National Statistics Office (NSO), marriage contract and my passport (mother) or my husband's passport whoever is processing or will accompany the infant as only parents can process if minor applicant. I told the DFA customer service officer that I have her Original registered birth certificate but not in SECPA as per NSO they can only provide it after 6 months of registration (which i cannot wait), so the officer told me that I can bring the original copy of the BC and have NSO officially authenticate it. So the next day, 2 weeks post natal from my CS, my sister accompanied me to the NSO office (previously located at City Savings Bank Financial Plaza along Osmeña Boulevard corner P. Burgos Street which is now in Gaisano South) we have to take a taxi as parking is very hard to find, it was so hot as the receiving area didn't have air-conditioner and the line was really long, when it was my turn, i explained to the officer that I need to process my infant's passport and DFA told me i can have it duly authenticated by NSO, the officer explained to me that I cannot submit the original copy but i have to have a certified true copy, he gave me instructions to photocopy the original Birth Certificate, bring it to the Cebu City Local Civil Registrar (CCLCR) and have it certified true copy then go back to NSO and look for him with the CTC Birth certificate. So my sister and i took a taxi and went to the CCLCR and followed what was instructed which we completed in 10 minutes. Went back to NSO and submitted the Certified True Copy, we were asked to come back 3pm to claim it. So we took our lunch and then at exactly 3pm we went to the same window and claim the Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR) and duly authenticated by NSO. The next day my sister, me and my daughter went to DFA and made used of the COURTESY LANE, we fill-up the form and then the guard let us in, we submitted the 3 requirements and we were out in less than 20 minutes. We opted for rush processing (7-10 working days) and we were able to claim her passport in less than 2 weeks. SO TO AVOID OTHER MUMS to go through the HASSLE of what i have been through below is the STEP by STEP in processing your infant passport application in Cebu. Did these steps on my 3rd child last November 2014. It was smooth and fast compared to my experience last 2013. It actually can be done in 1 day if you start early.
1. Photocopy the Birth Certificate that the hospital gave you and bring to the Cebu City Local Civil Registrar (CCLCR),

1. Photocopy the Birth Certificate that the hospital gave you and bring to the Cebu City Local Civil Registrar (CCLCR),
- a. Get request forms (RF) from Window 1 and fill-up data.
- b. Pay P50.00 at Window 3, they will give you a number
- c. Submit RF and OR to Window 2 for verification of ID
- d. Wait at the bench for your number to be called
- e. Get issued Certified True Copy (CTC) Birth Certificate (BC) at Window 10 when called
- a. Get AF and Queue Number from the Information Marshall and fill-up the required information
- b. Proceeds to the Screeners' Desk
- c. Pay P140.00 to the Cashier
- d. Forwards request for processing, give the CTC BC
- e. After an hour, get the authenticated BC
- a. Fill-up the application form provided by the guard and informed him you will be in the courtesy lane
- b. Submit documents for evaluation and processing
- c. Pay the passport fee of P1200.00 for express processing or P950.00 for normal to the cashier
- d. Submit Proof of payment, the officer will then take the photo of the infant and thumb mark, make sure he/she is awake.
- e. Check passport enrollment form for possible typo errors.
- f. Good to go home
- g. After 2 weeks or so (depends on your passport fee) Go back to the Releasing area to claim your passport, make sure to bring your receipt and a valid id. No need to bring infant. Again only the parents can claim, others even grandparents are not allowed.
